As your family physicians, we believe that the time to think about end-of-life decisions is while you are still healthy. If you have created a living will, have designated a health care proxy or have composed advanced directives, please make sure a copy of each is contained in your medical record in our office. If you have not yet thought about these issues, an organization called Choices in Dying has a web site with unbiased information about end-of-life decisions. Their web site includes approved forms for each state. You can print these forms directly from their web site, customize them for your own needs and desires, and sign them. We suggest you then give us a copy to be placed in your office records.
